Job description
Amedisys is seeking an RN case manager to join our home health and hospice team in the Roseburg area. This full-time role focuses on patient assessments, care planning, and collaboration with the care team to ensure high-quality, compassionate care at home.
You will develop individualized plans, make referrals as needed, and maintain progress records while teaching patients and families essential care techniques. On-call responsibilities are included as part of the duties.
