Job description
Your Health is seeking a Registered Nurse with hospice experience to service patients in the Blythewood area. The Nurse Case Manager will travel to patients’ homes and facilities to evaluate care needs and initiate telemedicine visits, ensuring quality care within regulatory guidelines.
The role requires coordinating care with providers, performing essential assessments, and documenting activities while working full-time with 12-hour shifts. A valid RN license and travel readiness are essential.
