Job description
Your Health Organization in Columbia, SC is seeking a Registered Nurse to serve our patients as a Nurse Case Manager. The role travels to patients' homes, long-term care facilities, and supports telemedicine visits to coordinate care.
The position requires reliable transportation, Monday–Friday 8–5, and travel within the Columbia area including Blythewood and surrounding towns. You will work with the care team to enroll patients, manage care plans, and document activities.
