Job description
Your Health Organization in South Carolina is seeking a Registered Nurse with Hospice experience to serve patients in Blythewood and Winnsboro. The Nurse Case Manager will travel to patients’ homes and facilities, coordinating with care teams to evaluate needs and initiate telemedicine visits.
This is a full-time, salary-based position with 12-hour shifts. You must have reliable transportation for daily travel and be licensed to practice as a RN in good standing.
