Job description
Amedisys Hospice in Glen Allen, Virginia is seeking a dedicated RN Case Manager to assess patients and collaborate with the care team to implement individualized plans for home-based hospice care.
Responsibilities include patient assessments, documenting progress, teaching families, and coordinating care across disciplines. Licensure, CPR, and a valid driver’s license are required along with at least one year of nursing experience.
