Job description
CHRISTUS VNA Hospice in Texas is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients at home and collaborate with a multidisciplinary team to promote physical, psychosocial, emotional, and spiritual well-being.
You will assess patient and family needs, implement treatment strategies, document progress, and educate on self-care while upholding compassionate, patient‑centered care.
