Job description
Superior Hospice is seeking a dedicated Hospice RN Case Manager to lead care for patients in the Greater Twin Cities area. You will manage an assigned caseload across the care continuum, focusing on comfort, pain control, and quality of life, while collaborating with a team of physicians, social workers, chaplains, and others.
You will conduct comprehensive nursing assessments, develop individualized care plans, and educate caregivers throughout the hospice journey.
