Job description
Gentiva is seeking a Hospice RN / RN Case Manager to lead personalized care at patients' homes or other familiar settings. You will assess, plan, implement and document care while guiding families through the hospice journey with compassion.
Responsibilities include collaboration with an interdisciplinary team, education for patients and caregivers, and ensuring regulatory compliance and quality in every visit. Travel to patient homes is required.
