Job description
Hospice of the Chesapeakes is seeking RN Case Managers to provide nursing care and case management for patients with life-limiting illnesses in their homes in Charles County, Maryland. This full-time role focuses on care continuity and quality, working with an interdisciplinary team including social workers, chaplains, CNAs, and hospice physicians.
Responsibilities include assessing patients during visits, leading the journey through illness, and coordinating goals with caregivers.
