Job description
ExcelaCare Hospice in Brownfield, TX is seeking an experienced RN Case Manager to administer hospice care in patients' homes and facilities, coordinating with a multidisciplinary team to ensure high-quality, compassionate care.
You will develop and implement individualized care plans, document patient status, educate families, and participate in on-call rotation. Current RN license and CPR are required, with travel locally.
