Job description
Bristol Hospice in Belfast, ME is seeking a dedicated RN Case Manager who will support patients and families through the hospice journey with compassion and clinical excellence. You will develop individualized care plans, monitor patient needs, coordinate with interdisciplinary teams, and document progress, while providing education and comfort during daily visits and care transitions.
