Job description
St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver 1:1, whole-person care to patients and families in their homes. You will coordinate individualized care plans with an integrated care team and support families through challenging decisions using empathy and clinical excellence.
The role requires an RN through an accredited program with 1+ years of acute care experience; hospice/home health experience is preferred. On-call duties and reliable transportation are required.
