Job description
Anchor Health is seeking an RN Case Manager to coordinate hospice care across patient homes and communities. The role requires an active CA RN license and strong interpersonal skills to support patients and families in terminal illness scenarios.
The Hospice RN will initiate and manage comprehensive care plans, collaborate with the interdisciplinary team, and ensure timely documentation and education for patient self-care at home.
