Job description
Gentiva Hospice seeks an experienced RN Case Manager to lead patient care in home and facility settings. You will perform assessments, develop plans of care, and coordinate with an interdisciplinary team to deliver compassionate hospice services.
Strong communication with patients, families, and providers is essential. The role requires an active RN license, at least 1 year in hospice/palliative care, and CPR certification.
