Job description
Advanced Health Care in Highland Heights, OH is seeking a Hospice RN Case Manager to coordinate and manage patient care in a hospice setting. You will provide skilled nursing services, develop and implement care plans, and collaborate with an interdisciplinary team to ensure comfort and dignity at end-of-life.
This full-time role requires strong clinical knowledge, excellent communication, ability to work independently, and reliable transportation for home visits.
