Job description
Coordinate compassionate, individualized hospice care by assessing patients, collaborating with the interdisciplinary care team, and developing plans that support each patient's needs. Maintain accurate care records, track pain and symptom management, teach patients and caregivers, and communicate regularly with the clinical team. Provide assigned on-call services.
