Job description
Amedisys is seeking an experienced RN Case Manager for full-time day shifts in the St Joseph/Elkhart territory. You will assess patients, develop and implement plans of care, and coordinate with the care team to promote patient health at home.
Responsibilities include supervising LPNs and HHAs, timely documentation, on-call duties, and ongoing clinical development. Qualified candidates have 1+ year RN experience, an active RN license, CPR, and a valid driver's license.
