Job description
St. Croix Hospice is seeking an experienced RN Case Manager in Minnesota to lead patient care on end-of-life journeys, coordinating with the hospice medical director, patient, family, and facility staff to implement and adjust the Plan of Care.
You will provide day-to-day nursing care, administer medications, maintain accurate records, educate families, and supervise care team members, including on-call duties evenings and weekends.
