Hospice RN Case Manager — Lead Compassionate Care

Job description

St. Croix Hospice is seeking a Registered Nurse Case Manager to lead the patient’s hospice care team and manage the Plan of Care in collaboration with the Hospice Medical Director, patient, family and facility staff.

You will provide day-to-day patient care, manage treatments and medications, and direct other care team members to meet changing patient needs in a home setting with focus on comfort and quality of life.
