Job description
LilyCare Hospice in southern New Hampshire is hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up. The role covers a southern New Hampshire territory, based out of Bedford, NH, with a rotating on-call schedule.
Responsibilities include assessments, care planning, symptom management, inter-disciplinary coordination, patient education, and meticulous EMR documentation.
