Job description
Heartland Hospice is seeking a dedicated Registered Nurse to join our team as an RN Case Manager. You will lead in-home patient care, document assessments, and collaborate with physicians and the hospice team to implement personalized plans of care.
Responsibilities include initial and ongoing assessments, education for patients and caregivers, and ensuring timely documentation while maintaining regulatory compliance. Prior hospice experience is preferred and a valid driver’s license is required.
