Job description
Gentiva seeks a dedicated Hospice RN Case Manager to lead care planning for patients at home or in supported facilities across Alabama. You will assess needs, deliver skilled nursing care, educate families, and coordinate with an interdisciplinary team to ensure compassionate, high-quality end-of-life services.
You will document visits, support caregivers, and participate in on-call activities, contributing to a supportive program focused on dignity, comfort, and patient-family education.
