Job description
Choice Health At Home is seeking a Hospice RN Case Manager (Registered Nurse) for its Greenwood Village hospice branch. The role centers on coordinating comprehensive nursing care plans for patients at home or in other care settings, ensuring comfort, dignity, and family support.
You will collaborate with interdisciplinary teams, perform assessments, develop and update plans of care, and provide education to patients and families. On-call coverage is part of the responsibilities.
