Job description
Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead patient care in home settings. You will assess patients, implement and adjust plans of care, and collaborate with physicians, LPNs, CNAs, social workers, and chaplains to deliver compassionate, person-centered end-of-life care.
