Job description
Gentiva is seeking a Hospice RN / RN Case Manager to deliver personalized hospice care in patients' homes or care settings. You will assess, plan, implement, and document nursing care while coordinating with an interdisciplinary team to ensure compassionate, compliant end‑of‑life services.
Strong communication and patient/family education are essential. Required RN licensure and at least 1 year of RN experience, with hospice or palliative care preferred.
