Cherish Hospice Corp in Canton, OH seeks an RN Case Manager to coordinate compassionate home-based care for patients and families.
This full-time role leads a multidisciplinary team, advocates for patients, develops care plans focused on quality of life, and documents the patient journey with empathy and precision.
You will travel across the Canton area to patients in homes, assisted living, long-term care facilities, and hospitals, delivering comfort and leadership wherever care is needed.
