Job description

Ascend Health in Garrettford, PA is seeking an RN Case Manager for our Hospice program. The role involves on-call coverage, patient visits, and coordinating end-of-life care with the healthcare team.

Requirements include an Associate's Degree in Nursing, a State Certified RN license, and at least 2 years of nursing experience (hospice preferred). A valid driver’s license and willingness to travel are required.
