Job description

Gentiva seeks a Registered Nurse Case Manager (RN) to lead in-home hospice care. As the primary contact, you will guide patients and families, document care plans, perform assessments, and coordinate with physicians, social workers, and the broader care team.

You will educate families on disease processes, symptom management, and end-of-life care while ensuring compliant, compassionate care and timely communication with the care team.
