Job description
Bloom Healthcare, a pioneering and employee-owned hospice practice, seeks a Hospice Registered Nurse Case Manager to coordinate care for homebound patients in the Littleton area.
Responsibilities include assessing needs, managing pain, educating families, and coordinating the interdisciplinary team across care settings. CO RN license and at least 1 year med-surg/ER/ICU experience are required.
