Job description
True North Consultants, Inc. in the Kansas City metro, MO is seeking a Hospice RN Case Manager for a full-time role.
You will coordinate and oversee patient care, develop and implement individualized plans of care with the interdisciplinary team, and manage symptoms while supporting patients and families. The ideal candidate holds MO RN licensure, CPR certification, a valid driver's license, and has strong clinical and interpersonal skills.
