Job description
Paycom - ATS in Grand Rapids, MN is seeking a Registered Nurse Case Manager (RNCM) to lead the hospice care team, developing and managing the Plan of Care in collaboration with the Hospice Medical Director, patient, family, and facility staff.
The RN Case Manager provides day-to-day patient care, administers medications, maintains records, and coordinates care with physicians and the interdisciplinary team to meet changing needs with a focus on comfort and quality of life.
