Job description
Silverado Hospice in Dallas, TX is hiring an RN Case Manager to coordinate and provide hands-on hospice and palliative care for patients and families across Dallas, Garland, and McKinney. You will develop plans of care, collaborate with the interdisciplinary team, and supervise field staff, ensuring high-quality end-of-life care.
The role requires an RN license with at least 1 year of RN experience (hospice preferred), ability to lift up to 50 pounds, and willingness to drive to patient
