Job description
Swan Hospice in New Jersey seeks an experienced Registered Nurse (RN) to join our hospice team. We provide care in skilled nursing facilities, assisted living facilities, and private homes, delivering compassionate, patient‑centered services.
The RN will oversee a primary caseload, perform comprehensive assessments, and collaborate with the Hospice Interdisciplinary Team to plan and revise care. An active New Jersey RN license is required.
