Job description
Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead patients and families through the hospice journey in their homes and community settings. You will perform comprehensive nursing assessments, develop and update plans of care, and coordinate with physicians, LPNs, CNAs, and social workers to ensure compassionate, high-quality end-of-life care.
Strong communication and education skills are essential.
