Job description
Gentiva Hospice seeks an experienced RN Case Manager to lead in-home nursing care for patients with life-limiting illnesses. You will assess patients, implement plans of care, and collaborate with physicians, LPNs, CNAs, social workers, and chaplains to ensure high-quality, compassionate care in patients' homes.
You will educate families, document care plans, manage symptoms, and advocate for patient comfort and dignity while adhering to regulatory standards.
