Job description
Gentiva Hospice is seeking a dedicated RN Case Manager to lead in-home hospice care, coordinating with physicians, social workers, and chaplains to deliver comprehensive plans of care. You will perform assessments, manage symptoms, and educate families to support comfort and dignity at end of life.
The role requires an active RN license and at least 1 year of nursing experience, with hospice familiarity preferred.
