Job description
Gentiva Hospice in Georgia seeks an experienced RN Case Manager to deliver in-home hospice care, assess patients, and coordinate with the IDG to create individualized plans.
You’ll educate families, manage symptoms, document care, and ensure regulatory compliance while supporting patients across home, assisted living, or residence.
Requires 1 year RN experience in hospice or home health; Georgia license; CPR; valid driver's license and reliable transportation.
