Job description
Heartland Hospice is seeking a dedicated Hospice RN / RN Case Manager to deliver personalized end-of-life care in patients’ homes. You will assess, plan, implement, and document skilled nursing care while coordinating with an interdisciplinary team to ensure compassionate and comprehensive services.
The role requires an RN license with CPR, at least one year of RN experience (hospice or palliative care preferred), and a valid driver's license.
