Job description
Heartland Hospice in Wisconsin is seeking a compassionate RN Case Manager to lead patient care in the home or facility setting. You will develop and review Plan of Care, perform assessments, and coordinate with physicians, clinicians, and social workers to deliver comfort-focused hospice services.
You’ll educate families, manage symptom control, document care accurately, and participate in on-call coverage. A valid RN license and 1+ year hospice or related clinical experience are preferred.
