Job description
Gentiva Hospice is seeking an experienced RN Case Manager to lead compassionate, home-based care for patients with life-limiting illness. You will perform comprehensive assessments, develop and adjust Plans of Care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure comfort and dignity in the patient’s home.
You will educate families, document care accurately, and participate in on-call activities, all while adhering to regulatory standards and quality
