Job description
Gentiva Heartland Hospice is seeking a Registered Nurse Case Manager to lead in-home hospice care, coordinating with an interdisciplinary team to deliver compassionate, patient-centered care. You will assess, educate families, and ensure plans of care are executed with respect for dignity and comfort.
The role requires an active RN license, at least 1 year of RN experience, and Hospice/palliative care exposure is preferred.
