Job description
Gentiva Hospice is seeking a compassionate Registered Nurse Case Manager to deliver in-home hospice and palliative care, guiding patients and families through end-of-life care with dignity, comfort, and empathy.
You will collaborate with physicians, LPNs, CNAs, social workers and chaplains to create and refine personalized care plans, perform assessments, educate families, and ensure accurate documentation while upholding quality standards and compliance.
