nWe are currently recruiting for a compassionate Hospice Registered Nurse (RN) Case Manager to reliably serve patients in small territories throughout Montgomery County and Bucks County, PA.
nWhat you'll do:
nAs a Jefferson Health at Home by BAYADA Hospice Nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home. You will:
nnnMaking home visits to clients in designated geographic territories.
n
nnPerforming assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
n
nnMonitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
n
nnFollowing up with, executing, and properly documenting doctors' orders.
n
nnOccasional on call after hours and/or weekends
n
n nWhat you should have:
nnnActive and current license to practice as a professional registered nurse (RN) in the state of PA.
n
nnOne year of hospice experience strongly preferred.
n
nnPrevious home healthcare experience welcomed, but not required.
n
nnAbility to travel to cases as assigned
n
nnA calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.
n
nnThe ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability .
