nHospice RN Case Manager
n$10,000 sign on bonus
nPosition Summary
nThe Hospice RN Case Manager provides compassionate, patient-centered nursing care to hospice patients and families. This role is responsible for assessment, care planning, symptom management, coordination of services, patient and caregiver education, and overall case management to ensure quality end-of-life care.
nKey Responsibilities
nnnManage a hospice patient caseload and serve as primary case manager.
n
nnComplete comprehensive patient assessments and develop individualized care plans.
n
nnCoordinate care with physicians, hospice aides, LPNs, social workers, chaplains, and other interdisciplinary team members.
n
nnManage pain and symptoms using evidence-based hospice and palliative care practices.
n
nnEducate patients and families on disease progression, end-of-life care, and available hospice services.
n
nnMaintain accurate and timely clinical documentation.
n
nnParticipate in interdisciplinary team meetings and care conferences.
n
nnSupervise Hospice Aides and LPNs, including required supervisory visits and care plan oversight.
n
nnSupport patients and families through grief, loss, and emotional transitions.
n
n nQualifications
nnnGraduate of an accredited nursing program.
n
nnCurrent RN license in the state of practice.
n
nnCPR certification required.
n
nnMinimum one year of acute care nursing experience required.
n
nnHospice or home health experience preferred.
n
nnStrong communication, assessment, and relationship-building skills.
n
nnValid driver's license and reliable transportation.
