Job description
Brighton Hospice in Indianapolis is seeking an experienced Registered Nurse Case Manager to plan, organize and direct hospice care. You bring nursing expertise with community health education experience and will build on community resources to tailor services for patients and families in their homes and communities.
You will assess needs, initiate and revise plans of care, and coordinate patient care for an assigned caseload.
