Job description
Suite Living Hospice Care in Minnesota is seeking an experienced RN Case Manager to join our hospice team in the Roseville area. You will manage a caseload of 10–15 patients, coordinating care across an interdisciplinary team to ensure comfort, dignity, and quality of life for patients and families.
This full-time role offers a Monday–Friday schedule with a localized northwest metro territory, mileage reimbursement, and an on-call rotation (5 days per month) to support seamless patient care.
