Job description
Grammar LLC is seeking a dedicated Registered Nurse Case Manager to support hospice patients and their families. You will coordinate patient care, lead nursing staff, and collaborate with healthcare teams to ensure seamless service delivery in the field.
Local travel across multiple counties is required, with standard M-F 8a-5p hours and on-call weekend coverage. Dedicated after-hours RN support is provided to handle urgent calls.
