Job description
Anchor Health is seeking a dedicated Hospice Registered Nurse Case Manager to coordinate hospice care for patients in their homes and communities.
The role requires CA RN licensure, CPR, and reliable transportation, with hospice experience preferred. The RN will develop and monitor care plans, educate families, and collaborate with the interdisciplinary team to ensure quality end-of-life care.
