Job description
Athena Home Health & Hospice is seeking a compassionate Hospice RN Case Manager to plan, organize, and direct home-based hospice care for patients and families in southern New England.
The role covers Litchfield County and western Hartford County. It’s a salaried position with weekly pay up to $49 per hour, Monday through Friday, 8:00 a.m. to 4:30 p.m., with opportunities for professional growth and comprehensive benefits.
