Job description
Athena Home Health & Hospice is seeking a compassionate Hospice RN Case Manager to plan, organize, and direct home-based hospice care for patients and families in Southern New England. This on-the-road role covers Litchfield County and western Hartford County with a salaried position and weekly pay up to $49 per hour.
